Appendix F: Appointment Change Form

The Appointment Change Form is used for the following circumstances: Promotion (involves $$), title change, reassignment, transfer, increase/decrease in % time only, regular AP or non tenure track apt for a current Postdoc Research Associate or Research Associate. Please consult EOA for further instructions on when to use this form.

Start a Workflow in HireTouch
Log into HireTouch, click on jobs, then Start a Workflow (located on the right hand side of the screen). Choose the Job Template that is appropriate for your proposed appointment. Hiring Request forms will be available in HireTouch at a later date.  PAPEs are included for each AP template. Templates you can choose are the following:

APs:
Academic Professional Appointment Change
Academic Professional Search
Academic Professional Search Waiver

Faculty Administrators:
Faculty Administrator Appointment Change
Faculty Administrator Search
Faculty Administrator Search Waiver

Faculty and Other Academics:

PAPE Only

After you choose the correct Job Template, Click "Continue". Fill in the required job details:

  1. Title of position- DEPT. Example: Coordinator of Research Programs- Psychology. For Faculty searches: Dept-title
  2. Chart and Org Code: The chart for everyone is 1, the org code is the first 3 digits of your organizational code followed by three zeros. A few units do not have three zeros- Amy Elli will tell you if you are an exception. Please enter the numbers in without any spaces or dashes (Example 1580000). If you enter in the code incorrectly, everyone in HireTouch can view your job information.
  3. The Organization Name will automatically populate after you enter your correct chart and org code.
  4. College Name: Liberal Arts and Sciences.

Click "Continue".

Do not change anything on this next page, scroll to the bottom and click "Continue".
You will be taken to the Forms tab for your Job.

f the PAPE is less than 3 years old and none of the duties or qualifications have changed, then upload the exisitng PAPE using the Forms tab. The following information is requested in the Apt Change

Form in HireTouch. Please gather the following information before beginning the Apt Change Form:

  1. Is the unit going through a reorganization?
  2. Will the apt change create a vacancy?
    If yes, will the vacancy be filled through a search?
    If no, will the position be eliminated?
  3. UIN, name, gender and race of proposed appointee
  4. Was proposed appointee hire through a search at any point in their employment at UIUC?

If yes, please list the search number.

  1. Is there a significant change in job duties or responsibilities for the proposed appointee?
  2. Is there a change in salary associated with the change in responsibility?
    List the Current and proposed salary
  3. List other dept employees with similar titles, duties and/or job qualifications
  4. Justification letter: address how the candidate was selected for this change in appointment over other employees, specifically those in the dept with similar titles, qualifications and/or job duties, and years of experience.  Please note that mid-year promotions with salary increases must be justified under extraordinary circumstances involving a clear change in duties, a salary inequity, or a counter offer.
  5. Proposed appointee's current resume or c.v.
